Membership in the Academy is by invitation only and will require a Proposer as well as a Sponsor.  Downloadable or printable PDF application forms are available below which can be used for direct online submissions or can be downloadable which after being completed can be faxed or e-mailed to the Admissions Chairperson.

Requirements for Active Membership

(a) The Applicant should be a qualified dental practitioner having maintained a license to practice dentistry from a recognized regulatory body or be a full-time educator in an accredited institution for a period of at least one year before being eligible for proposal to Active membership;

(b) The Applicant should be recognized in the dental profession as being a
practitioner of restorative and/or prosthetic dentistry or as being a full time educator in restorative and/or prosthetic dentistry;

(c) The Applicant should be willing to render his or her best efforts to stimulate
progress in their profession;

(d) The Applicant should be willing to promote the best interests of dentistry and
The Academy;

(e) The Applicant should have attended one scientific meeting as a guest;

(f) The Applicant should possess the potential for achieving the qualifications of
a Fellowship member including making a clinical essay, or case presentation to the Academy or a committee thereof.

Requirements for Life Membership

Life membership may be granted, at the discretion of the Council, to Active or Fellowship members in good standing who, because of age or health, have retired permanently from active practice and who has attained 70 years of age, and who have applied for transfer to Life membership status. The applicant shall have been a member in good standing for ten years preceding.

Requirements for Fellowship Membership
Fellowship Member status is by qualification. It is based on a 10-point merit system. Applicants may download the Application form and upon completion fax it to the Admissions Chairperson.

CARDP APPLICATION FELLOWSHIP

Requirements for Military, Faculty, Second Year Dental Grad and Technician Membership

(a) The Applicant should be a qualified dental practitioner or working in the dental field having maintained a license to practice dentistry from a recognized regulatory body or be a full-time educator in an accredited institution for a period of at least one year before being eligible for proposal to membership;

(b) The Applicant should be recognized in the dental profession as being a
practitioner of restorative and/or prosthetic dentistry or as being a full time educator in restorative and/or prosthetic dentistry;  or be working in the technical field of dentistry

(c) The Applicant should be willing to render his or her best efforts to stimulate
progress in their profession;

(d) The Applicant should be willing to promote the best interests of dentistry and
The Academy;

(e) The Applicant should have attended one scientific meeting as a guest;
